If you are planning to do so via IRS statements or business records, your time is running out!  Get your application mailed before June 30, 2009. Download your application at www.in.gov/pla/massage.htm. The State Massage Board has agreed to accept your AMTA membership papers as proof of business. Please be certain to have any submitted supporting documents notarized and signed by you. If you will be using the proof that you have graduated from an ICOPE or an out of State program with a minimum of 500 hours, before July 1st 2007, you will have until July 1, 2010 to get your application in. Have you filed a complaint with the Indiana Attorney General's office or the Cosmetology Board concerning Estheticians and Cosmetologists offering massage services? If so, it would be very helpful if you could provide a copy to Jennie Irving (765-748-1729 or jennie.irving@amtaindiana.org) ) as we have an appearance scheduled with the State Cosmetology Board in July. This would aid in documentation and history of concerns.
