If so, you need to download an application at www.in.gov/pla/massage.htm . Here you will see several options for application. If you graduated from a State approved program (or the equivalent from another State) you may still apply under the grandfathering option. Please read the instructions and Rules and Regulations of the Profession. An additional requirement after the above date is that you show proof of insurance listing the State of Indiana as an additional insured. Not having this will result in your application process being held up until you can show this coverage. To comply, AMTA members need to call the insurance carrier, HPSO, at 888-253-1474, and request an updated statement of coverage with Indiana added as an additional insured. This may then be copied, notarized and submitted. You may also call the National AMTA office at 847-864-0123 and ask them to help with this. Upon renewal of your membership, you may check a box that requests additional insured and this will be automatically done for you.

IF YOU DO NOT MEET THE FOLLOWING REQUIREMENT, YOU DO NOT QUALIFY FOR CERTIFICATION UNDER THE GRANDFATHERING PROVISION:

YOU MUST HAVE ENROLLED IN A MASSAGE THERAPY SCHOOL OR PROGRAM BEFORE JULY 1, 2007 THAT REQUIRED AT LEAST 500 HOURS OF SUPERVISED CLASSROOM AND HANDS ON INSTRUCTION. THAT PROGRAM MUST HAVE BEEN IN GOOD STANDING WITH A STATE, REGIONAL, OR NATIONAL AGENCY OF GOVERNMENT CHARGED WITH REGULATING MASSAGE THERAPY PROGRAMS AND YOU MUST HAVE COMPLETED THE PROGRAM BEFORE JANUARY 1, 2008.

 ALL GRANDFATHERING APPLICATIONS MUST BE POSTMARKED BEFORE JULY 1, 2010 TO BE ACCEPTED UNDER THE ABOVE PROVISION. PLEASE COMPLETE THE APPLICATION ON THIS WEBSITE AND INDICATE "GRANDFATHERING" AT THE TOP OF THE FORM.

IF YOU COMPLETED YOUR PROGRAM AFTER JANUARY 1, 2008, YOU MUST TAKE AN EXAM TO QUALIFY FOR CERTIFICATION. ACCEPTABLE EXAMS ARE LISTED BELOW UNDER "EXAMINATION INFORMATION".
